Hallucination

Language models produce likely text, not verified facts. When they lack knowledge, they may invent sources, figures, quotes or court rulings that sound real. The model is not “lying” on purpose – it has no built-in concept of truth.

Remedies include RAG with reliable sources, citations in answers, clear instructions to say “I don’t know” when unsure, and above all human review of important content.
